Family Caregiver
Greater Boston, Eastern MA, Cape Cod & The Islands
Are you concerned about your loved one’s ability to remain independent?
We support our clients and their caregiving families.
It’s natural to worry about our older loved ones. But when they begin to forget important tasks, can no longer physically manage basic activities, suffer a fall, are discharged from hospital back to their home or are diagnosed with a chronic or terminal illness, it’s difficult to be on top of every aspect of their care.
Why Family Caregivers Choose Best of Care
Best of Care works with your loved one and helps him or her maintain the highest possible level of independence. We begin with an overall evaluation of your loved one’s needs. We also look at the level of support you and your family are able to provide. From this evaluation we create a comprehensive plan of care.
The plan provides a day-to-day description of the services that are needed, what Best of Care will provide to your loved one and when, and how family members will be involved. The plan integrates and supports the care your loved one may receive from their physician or other healthcare professionals.
What’s more, our geriatric care management services act as an ongoing support system for your family to address your loved one’s evolving caregiving needs through long-term care plans, discuss difficult topics and complex issues, evaluate in-home care needs, evaluate alternative living options, and coordinate outside services.
Here are the many services we offer.
- Caregiver and Home Health Aide Services
- Help for Alzheimer’s, Dementia and Other Conditions
- 24/7 Care Services
- Geriatric Care Management
