What is the GUIDE Program?
GUIDE — Guiding an Improved Dementia Experience — is a new Medicare program designed to transform how people experiencing changes in memory and cognition receive care. Whether your family is in the early stages of noticing changes or further along in the journey, GUIDE meets you where you are.

Whole-Person, Coordinated Care — GUIDE wraps a full team of specialists — care navigators, clinical partners, and home care professionals — around the individual and their family.

In-Home Respite Support — Family caregivers receive dedicated respite hours with trained caregivers so they can rest and recharge while their loved one is in good hands.

A Dynamic, Evolving Care Plan — Each participant receives a personalized memory care plan that adapts as their needs change over time — nothing one-size-fits-all.

100% Medicare Covered — Qualifying Medicare beneficiaries receive all GUIDE program services at no out-of-pocket cost — no copays, no deductibles, no surprise bills.

WHO QUALIFIES
Eligibility Requirements
If you or a loved one has noticed changes in memory or cognitive function, you may qualify. Our team will conduct a no-obligation eligibility screening at no cost — anywhere in the US.
✓ Medicare Beneficiary
Must be enrolled in Medicare Parts A and B (Traditional Medicare).
✓ Noticed Changes in Memory or Cognition
A formal diagnosis is not required to start the conversation. If you or a loved one has noticed changes in memory, thinking, or day-to-day function, reach out — our team will help determine next steps.
✓ Community Dwelling
The individual must reside at home or in a community-based residential setting — not in a skilled nursing facility — at the time of enrollment.
✓ Not Already Enrolled in Hospice
Individuals currently receiving hospice care are not eligible for concurrent GUIDE enrollment.

Common Questions
Not necessarily. If you have noticed changes in memory, thinking, or day-to-day function, that is enough reason to reach out. Our team will help determine whether a formal evaluation is needed and guide you through next steps.
No. The GUIDE program’s 24/7 medical team layers on top of your loved one’s existing care — it is designed to work alongside their current doctors, not replace them. GUIDE’s medical team actively collaborates with your current physicians and specialists to coordinate and enhance the care your loved one is already receiving.
The GUIDE program currently requires Traditional Medicare (Parts A and B). Individuals enrolled in Medicare Advantage plans, Special Needs Plans, or PACE programs are not eligible at this time. If you are unsure which type of Medicare your loved one has, our team can help you find out quickly.
Most families are enrolled and receiving services within one to two weeks of their initial eligibility screening. Our team handles the application and coordinates directly with Medicare on your behalf.
Best of Care conducts eligibility screenings for families anywhere in the United States. If you qualify and live outside our direct service area, we will connect you with the appropriate GUIDE partner in your region at no cost to you.
